1989-2006 Mazda MPV Problems: 13 Known Issues

The solid-state cooling fan control module mounted in the engine bay of second-generation MPVs is a well-known failure point. When it fails, the radiator/condenser fans either run continuously the moment the key is turned (engine never reaches operating temperature, poor fuel economy, drained battery) or stop working entirely, which lets the engine overheat in traffic even though the coolant, water pump and thermostat are fine. Because the failure mode is intermittent at first, many owners chase thermostats and water pumps before finding the module. Mazda issued a special service program covering 2002-2003 vehicles with an updated module; owners report dealers occasionally reinstalled the original-spec part number rather than the superseded one, so the failure returned.

Second-generation MPVs route long steel coolant pipes down the underside of the body to feed the rear heater core. The rear wheel arch liner does not shield them, so tire spray blasts salt and grit directly onto the pipes. In salt-belt vehicles the steel corrodes from the outside in and eventually perforates, dumping coolant under the van - often with no visible leak in the engine bay, which sends owners looking for a head gasket. Owners on the MPV forums documented this as a design problem; Mazda changed the pipe material and added a urethane protector in later production, and issued a service bulletin with a warranty extension for corrosion of the rear heater pipe. A front coolant pipe was covered by an earlier corrosion campaign but the rear pipes were not included in it, which is the most common source of confusion.

About 13,030 MPV minivans were recalled because the linkage inside the rear door lock controller has an inappropriate shape that lets dust accumulate on it. With dust packed into the linkage, the lock mechanism cannot travel fully, so when the childproof locking system is engaged the door may only appear to be latched rather than fully latched. A door in that state can open while the vehicle is moving, with an obvious injury risk to rear-seat occupants. Mazda campaign number 82907; owner notification began July 26, 1999. This is a distinct failure from the later power sliding door motor and cable problems - it is a mechanical latch/linkage defect, not a power actuator failure, and it affects the childproof lock path specifically.

Late second-generation MPVs use a Takata passenger frontal airbag inflator with a phase-stabilized ammonium nitrate propellant and no desiccant. Long-term exposure to high absolute humidity, high temperature and repeated temperature cycling degrades the propellant so that it burns too aggressively on deployment, over-pressurizing and rupturing the metal inflator housing and firing shrapnel into the cabin. NHTSA campaign 18V402 covers 2004 MPVs nationwide and 2005-2006 MPVs in warmer, more humid states and territories, 18V404 (filed the same day) covers 2005-2006 MPVs sold or registered in the remaining colder states, and 19V781 covers 2004-2005 MPVs whose passenger inflators were previously replaced with inflators of the same design. Mazda escalated the remaining open Takata frontal airbag recalls to a formal DO NOT DRIVE advisory in August 2024, which applies to vans that were never repaired.

Mazda recalled 1989-1998 MPV minivans because the grease applied to the contact points inside the ignition switch can become electrically conductive over time. Once that happens, current can pass where it should not, and the switch can overheat. NHTSA campaign 15V674000 covers roughly 1,368,500 vehicles in total across every Mazda model included in the campaign, not the MPV alone. Because the MPV was sold across ten model years here, it makes up one of the longest year spans in the recall, so a large share of surviving MPVs on the road are candidates.

Alternator fails prematurely due to heat exposure from exhaust proximity. Battery drains and warning lights come on. Can strand vehicle if battery goes flat.

On MPVs equipped with factory front fog lamps, the bulb socket holder is pressed into the lamp housing. Heat from the bulb expands the socket holder and pushes it rearward; after repeated heat cycles combined with road vibration, the socket holder, bulb and wiring harness can separate from the lamp housing and drop loose inside the front bumper. A live, hot bulb sitting against bumper plastic is a fire risk. NHTSA campaign 03V053 covered improperly installed fog lamp bulb sockets, and a second campaign, 03V206 (Mazda campaign 1103F), covered the socket holder separating from the lamp case. The campaigns spanned MPV as well as other Mazda models built in the same window.

Certain MPV minivans built with V6 engines supplied by Ford Motor Company's Cleveland engine plant left the factory with fuel injectors that do not match the lower intake manifold they were installed into. The mismatch means the injector does not seal correctly in its bore, so pressurized fuel can leak into the engine valley and onto hot components, creating an under-hood fire risk. Because the mismatch is dimensional rather than a wear failure, it can leak from very low mileage and is not fixed by simply replacing O-rings. Owner notification began July 1999. This is a manufacturing/parts-supply defect specific to engines from that one plant, not a general MPV fuel system problem.

The second-generation MPV's automatic transmission (a 4-speed on 2000-2001 2.5L models, a Jatco 5-speed on 2002-2006 3.0L models) develops slipping, harsh shifts, and delayed engagement between 80,000-120,000 miles. The torque converter lock-up clutch wears, and the valve body solenoids stick. The transmission fluid often shows signs of degradation (dark color, burnt smell) well before problems are felt. Towing or heavy passenger loads accelerate wear.

Approximately 19,000 MPV minivans built between April 1, 1999 and November 14, 2001 and delivered without anti-lock brakes did not comply with Federal Motor Vehicle Safety Standard No. 135 for passenger car brake systems. The brake pipe layout split the hydraulic circuits in a way that, if one circuit suffered a partial failure, left the vehicle unable to meet the required stopping distance. This is a design/routing non-compliance rather than a wear item, so it does not announce itself with any symptom - the van brakes normally until the moment a circuit leaks, and then the remaining circuit is inadequate. Owner notification began January 15, 2002. This affects only non-ABS vans; ABS-equipped MPVs were not included.

Power sliding door motor, cable, or latch mechanism fails. Door may not open, not close completely, or reverse direction randomly.

Second-generation MPVs carry three catalytic converters: a main converter plus two close-coupled 'pre-cats' bolted right at the exhaust manifolds, one on the radiator side and one on the firewall side. Sitting that close to the ports, the pre-cat substrates run extremely hot and break down. The immediate symptom is a catalyst efficiency code - P0420 on earlier vehicles, P0421 (warm-up catalyst, bank 1) on Mazda's later coding - but the real hazard is that a disintegrating pre-cat can shed ceramic substrate which, on an engine that has any intake pulsation or a leaking exhaust valve, can be drawn back toward the cylinder and score the bore. The failure is almost always downstream of something else: a persistent misfire, a leaking injector, or an oil-consumption problem cooking the catalyst. Owners commonly replace the rear O2 sensor first and get the code back within a drive cycle.

The 3.0L AJ-DE V6 used in 2002-2006 MPVs (Mazda's version of the Ford Duratec 30) develops chronic external oil leaks as the RTV and gaskets age. The two dominant sources are the timing chain cover, where the sealant between the cover, block and heads gives up, and the valve covers, including the rubber grommets around the valve cover bolts, which split and weep. A third, less obvious source is the seal behind the water pump pulley area at the rear of the cam drive. The timing chain cover job is disproportionately expensive on this engine because all of the front-of-engine accessories, both valve covers and the oil pan must come off (along with the right halfshaft and crossmember) to extract the cover in the tight transverse bay. Left alone, oil drips onto the accessory belt and exhaust and produces the burning smell owners report.
